Targeted Case Management services assist individuals in gaining access to needed medical, social, educational, and other essential services. Our dedicated case managers work closely with clients to assess their specific needs, develop a comprehensive care plan, and coordinate referrals to community resources. We provide ongoing monitoring and follow-up to ensure clients are successfully connected to the support systems necessary for their overall well-being and stability.
A IMTS team member meets seriously emotionally disturbed children and their families where they are to assist with building life skills to ensure they can be successful in school, communities, social outlets and other aspects of the individual's life.
